Developed and implemented a nurse-led, person- driven model. Patients were invited to reflect on “what matters?” and then set a series of personal goals for life and health. Elena created training program, worked with product team, hired interdisciplinary team of clinicians and implemented the model at the practices. The model was successful from the patient and clinician perspective and provided team critical insight into financing whole person, team-based care models.
